  "abstract": "Introduction The 3rd edition of the International Classification of Sleep Disorders includes Catathrenia as a sleep disordered breathing (SDB) condition (Thorpy, 2017). The key catathrenia characteristic is a loud vocalisation, often described as a groan/moan (Martel et al., 2024). A further important feature of catathrenia on a sleep study is, no associated oxygen desaturations with respiratory events (Martel et al., 2024). Catathrenia prevalence has historically been reported as approximately 0.54% of adults, however, this is likely underdiagnosed with cases being mistaken for other types of SDB (Iriarte et al., 2015).Case History A 53-year-old female was referred for a sleep study with symptoms of an increase in snore, witnessed apnoeas and unrefreshing sleep. Following manual analysis of a limited polysomnography (LPSG) sleep study, an apnoea hypopnoea index (AHI) of 23.5/hr and oxygen desaturation index (ODI) of 3.5/hr were recorded. Most apnoeas were atypical central events. Significant audio data presented a clicking groan with apnoeas. The patient history and the sleep study results aligned with a diagnosis of typical catathrenia. The patient was initiated on positive airway pressure (CPAP) therapy and titrated to 12 cmH2O. A further LPSG was carried out while the patient was using CPAP which showed an AHI of 9.6/hour and ODI of 0.9/hour. The patient reported overall symptomatic relief with CPAP. Their husband also reported reduced catathrenia events. Further CPAP adjustments were declined by the patient for comfort reasons.Discussion Although the AHI is sub optimal, this is due to pressure intolerance and highlights the importance of symptomatic improvement, especially in the presence of a low ODI. This case study suggests CPAP remains an appropriate treatment for catathrenia. With more patients being screened for SDB, it is important for sleep scientists to be aware of the symptoms, presentation and treatment options for catathrenia to improve patient experience.",
