  "abstract": "Introduction Current guidelines advocate either Continuous Positive Airway Pressure (CPAP) or non-invasive ventilation (NIV) in Obesity Hypoventilation syndrome (OHS), depending on the severity of hypoventilation, to improve long-term outcomes (NICE guidelines NG202, 2021). Timely treatment of OHS with CPAP reduces hospitalisation and is cost-effective (Masa et al., Thorax, 2019).Aim We analysed the efficacy of CPAP therapy in patients with OHS irrespective of the severity of obstructive sleep apnoea (OSA) over 4 years (January 2021 to December 2024).Methods Retrospective analysis of 41 patients with OHS initiated on CPAP in a tertiary hospital using electronic case notes.Results 41 patients with OHS were started on CPAP. The mean age was 57.3 years, and the age range was 25 to 86 years. 24 % (n=9) of patients had mild or moderate OSA; one patient in this subset was switched to NIV. Compliance was good (>4 hours) in 23 patients (58.9%). The mean Apnoea Hypopnoea Index (AHI) and Epworth Sleepiness Scale (ESS) on initiation were 62.4 and 10.3, respectively. The mean body mass index (BMI) and bicarbonate on initiation were 45.8 and 29.5. The mean pCO2 on initiation and follow-up was 6.62 and 5.92. The mean AHI and ESS during follow-up were 4.83 and 4.68, respectively (table 1). 5 patients were switched over to NIV during follow-up, and 4 of them had a BMI >40. 14 patients were admitted to the hospital during follow-up (table 2).Discussion CPAP improved daytime hypercapnia, AHI, and ESS. According to the guidelines, CPAP is an effective treatment modality for patients with OSA/OHS and should be considered as the first-line therapy, given its cost-effectiveness, low labour intensity, and monitoring requirements compared to NIV. Our study noted that higher BMI, smoking history, and opioid use increased the risk of NIV. A larger study will provide more power to this evidence.Abstract P78 Table 1Abstract P78 Table 2",
