  "abstract": "Introduction OSA is strongly associated with AF. Treating OSA may improve outcomes for these patients. 1 We want to examine if those who are treated for OSA with CPAP has successful cardioversion.Methods We analysed 50 AF patients from October 2023 to December 2024 referred for investigation of OSA from new AF and cardioversion clinic.Results Of the 50 patients, 70% had electrical cardioversion. Of the remaining 15, 13 had paroxysmal AF or were not suitable for cardioversion. 2 declined cardioversion.Of the 50 patients, 90% attended sleep assessment. 4 did not attend and 1 declined.100% were diagnosed with OSA. 22% mild, 36% moderate and 42% severe. 40% of the mild were treated together with moderate or severe OSA patients with CPAP.Amongst 35 who underwent electrical cardioversion, 15(43%) had a successful procedure.Amongst 15 successful cardioversions, 53% were established on CPAP prior to cardioversion and 47% declined or couldn’t tolerate CPAP. 60% had hypertension, 47% had BMI>30 ;27% had BMI >40 . 33% had severely dilated left atria (LA). 20% of the successors had of raised BMI, hypertension and severe LA dilation. 33% of successful cardioversions were on anti-arrhythmic therapy before cardioversion.100% on CPAP prior to cardioversion and anti-arrhythmic drugs had successful cardioversions.Amongst 20 failed cardioversions, 55% were offered CPAP. 10 accepted CPAP. All 10 patients were cardioverted before they started CPAP. 100% of this group had BMI>30 (50% had BMI >40) & 55% had hypertension. 30% had severely dilated LA,15% had hypertension &raised BMI and severely dilated LA. 60% had hypertension & raised BMI.Discussions Treatment of OSA with CPAP is important for successful cardioversion of AF. Besides OSA, other risk factors for failed cardioversion include raised BMI, hypertension and severely dilated LA.Reference Gami AS, et al. Association of AF and OSA. Circulation. 2004;110:364–7.",
