  "abstract": "Introduction Assessment of paediatric sleep related breathing disorders (SRBD) typically requires polysomnography (PSG) with transcutaneous CO 2 (TcCO2) monitoring, most often performed as attended in-laboratory studies. However, these traditional approaches can be particularly challenging for children, especially those with neurodevelopmental conditions or multiple comorbidities. Advances in remote monitoring now allow unattended studies to be conducted within the home environment. These methods have the potential to improve patient comfort, better reflecting more natural sleep patterns, and expand diagnostic capacity by reducing the need for overnight attendance by sleep specialists. The aim of this review was to evaluate home-based service delivery for paediatric patients undergoing TcCO2 monitoring with both polygraphy or video-polysomnography and to correlate clinical indications with the diagnostic outcomes obtained.Methods We conducted a retrospective analysis of paediatric referral for private sleep studies, reviewing all paediatric patients who underwent home-based sleep studies from 2024 to the present (2025). We assessed patient tolerance and study feasibility, as well as study accuracy, device performance, and any signal loss that could affect diagnostic quality. For clinical outcomes, we evaluated the correlation between the initial clinical indication and the diagnostic findings, along with any subsequent initiation or modification of therapy based on study results.Results Retrospective data from 127 paediatric patients were analysed, with results in the final stages of preparation.Discussion Adding TcCO 2 to home video-polysomnography is a feasible and accurate method to evaluate sleep-related breathing disorders (SRBD) in paediatric patients, regardless of developmental status or comorbidities. Preliminary evaluation of TcCO2 measurements suggests a low rate of artefacts and technical problems, with strong alignment between clinical indications and diagnostic outcomes. Conducting these studies in the home setting may further enhance patient comfort and capture more representative sleep patterns.",
