  "abstract": "Introduction The 2023 American Academy of Sleep Medicine (AASM) scoring manual recommends that children over 13 years old with adult habitus may be scored using adult criteria. We aimed to compare paediatric and adult scoring in adolescents referred to a tertiary respiratory centre for evaluation of sleep-disordered breathing. The hypothesis is that adult criteria would detect fewer respiratory events than paediatric criteria.Method Cardiorespiratory polygraphy of 40 adolescents aged 13-18 years, self-ventilating in room air, were reviewed. Respiratory events were scored using AASM 2023 (version 3) paediatric and adult scoring criteria.Result Cardiorespiratory polygraphy of 25 male 15 female, mean age 14.8 year were analysed. Median (range) paediatric AHI 3.3/hour (0-45.9), adult AHI 2.5/hour (0-45.9). Reasonable agreement between paediatric and adult AHI ( figure 1). Scoring of obstructive hypopnea had greatest variability. More patients were diagnosed with OSA using paediatric scoring at thresholds of AHI ≥ 1.5/hr and AHI ≥ 5/hr (table 1), but differences were not statistically significant. Respiratory rate during quiet sleep is the only significant predictive factor for the difference between paediatric and adult AHI scoring. Regression coefficient = 0.09, p = 0.018. Age, sex, weight, height and BMI were not significant predictors.Discussion Adult and paediatric scoring criteria can be used in adolescent 13-18 years old. The difference in the scoring did not result in a significant change in diagnoses. Our cohort represents complex patients with underlying comorbidities that would affect their cardiorespiratory status. Breathing rate during quiet sleep rather than habitus would be a better guide in determining the applicability of scoring criteria. Further study is required to understand the developmental changes of sleep disordered breathing in adolescents and the clinical implications of various diagnostic cutoff.Abstract P61 Table 1McNemar test for paediatric versus adult scoring.
