  "abstract": "Aim To ensure that 100% of administered chemotherapy is accurately documented within the electronic prescribing system, Paediatric ChemoCare, across seven sites in the South West region—including one Primary Treatment Centre (PTC) and six Paediatric Oncology Shared Care Units (POSCUs).Method A retrospective audit was conducted from 5th to 9th May 2025, reviewing all chemotherapy administrations recorded on the Paediatric ChemoCare system. All patients who received systemic anti-cancer therapy and associated supportive care during this period were included. Compliance criteria required two electronic signatures to initiate each chemotherapy agent and one signature to confirm completion.Results The PTC administered the highest volume of chemotherapy, with 67 recorded encounters—defined as any patient receiving one or more chemotherapy administrations on a given day. Of these, 33 administration episodes were found to be incomplete. In every case, the start of infusion was recorded, but documentation confirming the completion of the infusion was absent.Among the six POSCUs Five achieved full compliance, with a combined total of 22 chemotherapy encounters. One regional hospital recorded five chemotherapy encounters, two of which lacked complete documentation.Conclusion The audit highlights ongoing challenges in achieving complete and accurate electronic documentation of chemotherapy administration, particularly at higher-volume centres. Incomplete records were due to omission of infusion stop times, despite initiation being correctly documented.To address this, all relevant centres have been notified. Moving forward, nursing staff will receive targeted reminders—delivered during safety briefings—highlighting the importance of signing off both the start and completion of chemotherapy. Additionally, it is recommended that the nurse in charge verify that all chemotherapy administrations are fully documented during each shift.A re-audit is recommended in 12 months to assess the impact of these interventions and to support continuous quality improvement.",
