  "abstract": "Background UK Emergency departments (EDs) are operating under significant pressures – rising patient demand and complexity, limited capacity, and workforce shortages. A significant contributor is the growing documentation workload. Solutions that can alleviate this administrative burden without compromising care quality and ED efficiency are needed.Methods This NHS-led service evaluation assessed ambient voice technology (AVT) in a tertiary ED in London (between November 2024 and April 2025). Three performance indicators: patients seen per shift, time to first documentation, and time from clinician assignment to patient readiness, were assessed. Comparison was made between baseline data from the preceding three months. Findings were triangulated with economic modelling by the York Health Economics Consortium.Results 4,664 patient encounters were analysed across 24 clinicians. With AVT there was a 13.4% increase in patients seen per shift (p=0·00018); 51.7% reduction in time to first documentation (p=0·0067); and 7.5% reduction in time to clinical readiness (p=0·0024). Clinicians reported improved experience, reduced cognitive load, and greater efficiency. Independent modelling showed if one additional patient was seen per shift per clinician, this equates to £270.93 added capacity per day. Scaling nationally (11,055 ED clinicians in England), AVT use could yield 9,259 more attendances each day, saving £176 million in documentation time and unlocking £658 million in capacity annually.Interpretation AVT significantly improves emergency care productivity, documentation efficiency, and clinician experience. The technology supports NHS efforts to address demand, reduce waiting times, and enhance staff wellbeing.
