  "abstract": "Background and Purpose Young people are usually referred to the Osteogenesis Imperfecta (OI), or brittle bone team at GOSH at an early age. From around the age of 12, the team begins to prepare young people and their carers for healthcare transition into adult services. Some young people have experienced complex social care involvement or they have experienced abuse in childhood, requiring careful thought to ensure they continue to receive ongoing individualised care and support as young adults. Consideration is needed as to how information is shared with adult services, empowering young people by placing their agency and protection at the centre. Careful planning is needed to ensure continuity of care for potentially vulnerable young people. This is transitional safeguarding.Methods The OI team presents two case studies demonstrating a transitional safeguarding approach.Results Patient 1 is a young person with severe OI, complex health needs, and limited verbal communication. He is known to a number of services including the Children with Disabilities social care team. We wanted to ensure that the young person’s voice was heard and that he could develop independence through the transition process. Through careful multiagency work involving the young person and family, a clear transition plan was agreed including continued support from adolescent and adult services to ensure the young person transitioned safely.Patient 2 is a young woman with mild OI who experienced abuse in childhood. Through considered discussion with her, a transition plan was agreed. The team respected her autonomy while advocating for continue access to additional support.Discussion/Conclusion The GOSH OI team plans to continue collaboration with other NHS trusts to develop a joined up approach to transitional safeguarding between paediatrics and adults services.",
