  "abstract": "Introduction The corticospinal and intracortical changes associated with pain in hip osteoarthritis are unclear. Our objective was to explore whether corticospinal and intracortical excitability were associated with hip-related symptoms and functional limitations.Materials and Methods We performed a cross-sectional study of people with hip osteoarthritis (confirmed with clinical assessment and magnetic resonance imaging). Symptoms and functional limitations were quantified via International Hip Outcome Tool (iHOT-33). Transcranial magnetic stimulation assessed quadriceps corticospinal and intracortical excitability via active motor threshold [AMT, percentage of maximal stimulator output (MSO)], motor evoked potential (MEP120, mV elicited from 120% AMT), short-interval intracortical inhibition (SICI) and intracortical facilitation (ICF). With iHOT-33 (symptoms and functional limitations) as the dependant variable, we performed two generalised linear models (Model one: age, AMT, MEP120, SICI; Model Two: age, AMT, MEP120, ICF) to explore the association of corticospinal and intracortical excitability with symptoms and functional limitations.Results We included 34 participants (24 female, 10 male) with a mean (standard deviation, SD) age of 59.4 (8.0) years. Participants had a mean (SD) iHOT-33 (symptoms and functional limitations) of 55.5 (20.1), AMT of 44.7 (8.2) %MSO, MEP120 of 1.04 (0.66) mV, SICI of 0.72 (0.27), and ICF of 1.27 (0.45). AMT was associated with symptoms and functional limitations (β=1.176, 95%CI=0.180–2.172, p=0.021), however MEP120 (p=0.418), SICI (p=0.602), and ICF (p=0.924) were not.Conclusion The relationship between corticospinal and intracortical excitability with pain in hip osteoarthritis is complex. We have demonstrated a greater AMT (lower corticospinal excitability) is associated with less hip-related symptoms and functional limitations.",
