  "abstract": "Purpose The aim of the study is to evaluate the prevalence of herpes simplex virus (HSV) on the donor corneal tissues linked to the HSV’s donor history and the endothelial density count.Methods A retrospective analysis was performed on a prospective database of FBOV donor tissues collected from August 2024 to September 2024. A total of 56 donors were tested for HSV type 1 IgG Ab (HSV1) and HSV IgM Ab (HSVM) and the HSV history was collected. For 108 ocular tissues a PCR of the culture sediment was tested for HSV-DNA and information on endothelial density was collected.Results From interviews with family members or the family doctor, it emerges that 56% of the donors have a negative history of HSV type 1, 23% a positive history of HSV type 1 and 21% don‘t remember. 14 of the 108 tissues (13%) are from donors with negative HSV1, negative HSVM and negative HSV type 1 history, 83 (77%) are from donors with positive HSV1 and negative HSVM, of these 43 tissues (52%) are from donors with a negative history of HSV type 1, 21 (25%) with a positive history of HSV type 1 and 19 (23%) don’t remember. From the 108 tissues 11 (10%) are from donors with positive HSV1 and positive/doubt HSVM, of these 4 tissues (37%) are from donors with a negative history of HSV type 1, 3 (26%) with a positive history of HSV type 1 and 4 (37%) don’t remember. All the PCR of the culture sediment tested for HSV-DNA of each tissues are negative for all the 108 corneas. 88 of the 108 corneal tissues (75%) were allocated for transplant with a medium endothelial density count over than 2400 cell/mm2.Conclusions This preliminary study suggests that the current donor screening procedure, and the risk-benefit ratio of HSV type 1 transmission, is acceptable but the role of donor cornea tissue in HSV type 1 transmission and disease need more additional investigation.",
