  "abstract": "Purpose Usually, corneas harvested in eye banks are used for keratoplasties: penetrating, anterior and posterior types, scleral tissue is used mainly in plastic surgery, however quite rarely in some clinical cases treated surgically, ocular tissues are applied in non-standard ways. The purpose of this study was to analyze a non-typical use of ocular tissues in the surgical management of these cases.Material From January 2010 to October 2013 36 patients were admitted to our department with: non-traumatic corneal perforations or descemetocoele (10 patients) extensive corneal bacterial ulcer (5 patients), severe viral ulcers (5 patients), very high intraocular pressure unresponsive for pharmacological treatment (20 patients), dry eye syndrome rheumatoid arthritis (15 patients).Methods In cases of perforated corneas due to extensive bacterial ulceration, penetrating keratoplasty was performed with a large diameter (11,5 mm) graft. In cases of corneal perforation due to severe dry eye syndrome – different grafts of donor corneal tissue were used: full thickness mini graft (4–5mm in diameter), anterior graft (lamellar), posterior mini graft (lamellar), anterior (lamellar) with an irregular crescent shape. In cases with high intraocular pressure unresponsive for pharmacological treatment, Ahmed valves were implanted. The anterior portion of the Ahmed valve tube was covered with a donor scleral graft secured with 7/0 sutures. In cases of severe descemetocoele or threats to the integrity of the eyeball, epikeratoplasty (tectonic graft, full-thickness corneoscleral button) was performed.Results In cases of corneal perforations 9 corneas were successfully healed, and in 8 patients showed improved visual acuity. In epikeratoplasty cases the integrity of the eyeball were achieved in all the patients; in one case during two-years follow up additional surgical treatment was necessary due to progressive corneal melting. Case of extensive bacterial ulcer and large-diameter penetrating keratoplasty graft survived at one-year follow up – visual acuity improved. Cases following Ahmed valve implantation – the valves were effectively protected from exposure by donor scleral tissue during one to two years follow-up. Intraocular pressure decreased in 18 cases.Conclusion Eye tissues harvested in eye banks can be successfully and effectively used in a non-standard way in various circumstances in ophthalmic surgery.",
