  "abstract": "Background/Objectives To explore the demographic and clinical features of cognitive super-agers using three different measurement approaches and determine which definition best explains variance in outcomes.Methods Super-agers defined as those with performance in ≥1 cognitive domain comparable to adults in their 30s-40s (SA1), superior performance for age (SA2) and no cognitive decline over time (SA3) were compared with typical agers in a sample of 1611 adults aged 65–106 from the Sydney Memory and Ageing Study, Older Australian Twins Study and Sydney Centenarian Study. Logistic regression was used to create a predictive model to determine which variables were important for each definition. Separate generalised linear models were used to determine how much of the total variance in outcomes could be explained by each of the definitions.Results Compared to typical agers, SA1 (n=459) and SA2 (n=236) had more education, higher occupational complexity, greater current activity levels, less hypertension and lower depression scores. SA3 (n=266) had higher physical activity levels, lower depression scores and less hearing impairment. All three cohorts had higher brain volumes and less white matter disease. Super-ageing status was associated with less functional impairment (10.1–16.1%, p <0.001) for all cohorts with strongest associations with executive function and memory abilities. Faster walking speed was seen for SA1 (b=0.05, p<0.001) and SA2 (b=0.06, p<0.001) and better psychological wellbeing (5–6.5%, p=0.001–0.003) with strongest associations with memory and language abilities.Conclusion Education, later life activity, supporting hearing and mental and vascular health may promote super-ageing which is associated with positive outcomes.",
