  "abstract": "Background/Objectives Carpal Tunnel Syndrome (CTS) is the most common entrapment neuropathy. The role of electrophysiological diagnosis of CTS is debated. The current study aimed to determine the relationship between the referral of clinical CTS and its subsequent electrodiagnosis, as well as factors that predict a positive CTS electrodiagnosis in a larger cohort of patients than previous studies.Methods Participants were prospectively recruited from those referred with a clinical suspicion of CTS to a neurophysiology laboratory between May 2020 and April 2021. Patient demographics and symptom characteristics were documented prior to electrodiagnosis. Nerve conduction parameters and electrophysiological CTS severity were subsequently recorded. Severity was graded as no CTS, mild, moderate and severe CTS.Results Of 250 patients analysed, 77% (n=193) were confirmed to have electrodiagnosis of CTS. Age (p=0.015) and female sex (p=0.05) were associated with CTS electrodiagnosis, with age associated with higher electrodiagnostic severity (moderate, p=0.004; severe, p=0.001). Predictors of electrodiagnostic outcome included age (p=0.005; OR 1.026, CI 1.009–1.050), median nerve distribution of symptoms (OR 0.409, CI 0.178 to 0.940), nocturnal symptoms (p=0.023; OR 2.736, CI 1.146–4.532), and symptomatic dominant hand (p=0.02; OR 2.357, CI 1.140–4.874).Conclusions The current study has demonstrated that approximately three-quarters of patients referred for electrophysiological testing with a clinical diagnosis of CTS have an electrodiagnostic diagnosis. Several pre-test clinical variables including age, sex, hand dominance, median nerve distribution, and nocturnal timing of symptoms may be used to assist the electrodiagnostic referral process.",
