  "abstract": "Background Anti-amyloid monoclonal antibodies are the first disease-modifying therapies (DMTs) for Alzheimer’s disease (AD). Participants were excluded on MRI grounds if they had severe white matter hyperintensity (WMH) burden, >4 cerebral microbleeds (CMB), prior stroke, or areas of cortical superficial siderosis (CSS). We aimed to establish potentially eligible patients via a single centre retrospective audit of patients presenting to the Royal Melbourne Hospital between 21st April 2019 and 21st April 2023. Additionally, we examined reports to determine if current MRI brain scans could be utilised to screen patients for eligibility.Methods We identified patients aged>50 years using Electronic Medical Records and ICD-10 coding for dementia and/or AD with MRI brain: FLAIR sequences to grade WMH Fazekas score; SWI sequences to identify CMB/CSS. MMSE and other cognitive screens, comorbidities, CSF analysis and FDG-PET were collated where available.Results We identified 752 patients. 171 of the first 200 patients (median age 77 years, 50.3% female) had MRIs containing FLAIR and SWI sequences. Twenty patients were eligible for donanemab or lecanemab (11.7%) based on reported trial criteria. CMBs count≥5 were reported in all patients (100%). Prior stroke, structural lesions, CSS and WMH/Fazekas scores were reported less frequently (90%, 68.6%, 1.5% respectively).Conclusion Only a small number of people with AD were eligible for current anti-amyloid therapies under published safety inclusion criteria. Standardising current MRI sequences and reporting to fulfil MRI inclusion requirements would enable more accurate pre-screening of potentially eligible patients. Multidisciplinary clinical education will be essential for safe implementation of AD DMTs.",
