  "abstract": "Background Multinodular and vacuolating neuronal tumour (MVNT) is a WHO Grade 1 tumour described in 2013. A variety of presentations and characteristic neuroradiological findings are reported. This study reviews a single-centre experience in MVNT.Methods A retrospective review of patients with suspected MVNTs identified on radiological studies at a single tertiary referral hospital in Sydney, 2010–2023. Demographic information, clinical presentation and radiological features were recorded.Results Between 2010–2023, 19 patients had lesions identified on MRI consistent with MVNT. Median age was 48 years. 53% were female. Clinical indications for imaging included headache (21%), seizures (11%), cognitive impairment (11%), tinnitus (1/19), hearing loss (1/19), aphasia (1/19), tremor (1/19), visual field loss (1/19), follow up for PRES (1/19), mastoid pain (1/19), screening for metastatic disease (1/19). All lesions demonstrated characteristic MRI features i.e. circumscribed cluster of T2/FLAIR hyperintense nodules in subcortical white matter +/- overlying cortex in cerebral hemispheres apart from two patients whose lesions were in the cerebellum. Distribution breakdown - temporal lobe (26%), parietal lobe (21%), frontal lobe (21%), occipital lobe (16%), limbic lobe (5%), cerebellar vermis extending into cerebellar hemisphere (11%). All lesions involved subcortical white matter. Six lesions (32%) also involved the overlying cortex.Conclusions Nineteen patients with characteristic neuroradiological findings of MVNT are described, with their clinical presentations. Long term radiological follow up (average 4.7 years) was available for most patients, confirming stability. This data supports MVNT is an indolent lesion that does not require biopsy or invasive intervention, and can be monitored radiologically.",
