  "abstract": "Background The Yellow Card scheme has served as the foundation of UK pharmacovigilance for 60 years, yet it operates as a fundamentally reactive system. Adverse drug reactions (ADRs) are reported only after patient harm occurs. However, pharmacogenomics now enables prediction of drug toxicity and efficacy at the individual level, exemplified by CYP2C19-mediated clopidogrel resistance and dihydropyrimidine dehydrogenase (DPD)-related fluoropyrimidine toxicity.Objective This manuscript examines how genomic integration can transform the Yellow Card scheme from passive surveillance to predictive safety monitoring, with particular focus on the Medicines and Healthcare products Regulatory Agency Yellow Card Biobank initiative.Key evidence Currently, approximately 30% of the UK population carrying CYP2C19 loss-of-function alleles receive ineffective clopidogrel therapy, while 5%–30% of patients exposed to fluoropyrimidines historically experienced life-threatening toxicity—now largely preventable through DPYD genotyping. The Yellow Card Biobank now links ADR reports directly to genomic sequencing, beginning with two pilot signals: allopurinol-associated Stevens-Johnson syndrome (HLA-B*58:01) and direct oral anticoagulant-related bleeding.Proposed innovations Three practical modifications are presented: (1) integration of a genomic flag within Yellow Card reporting infrastructure to leverage existing National Health Service genomic data from the 100 000 Genomes Project, (2) outcome-triggered automated reporting from electronic prescribing systems to capture ‘near-miss’ prevention events and (3) structured family history collection to identify familial clustering patterns suggestive of germline genetic causes.Conclusion Transitioning from reactive to predictive pharmacovigilance requires upgrading safety systems to capture genomic data systematically. This approach protects patients prospectively rather than retrospectively, representing the cornerstone of next-generation medicine safety.",
