  "abstract": "A female in her 20s presented to the emergency department with breathlessness and increased salivation for 12 hours. Her caregivers reported twitching of facial muscles and her hands. She also had altered mentation for 3 hours. She had no prior illness, trauma or bronchial asthma and denied any history of substance abuse. On examination, she was drowsy and had bradycardia, tachypnea and hypoxia. The Glasgow Coma Scale score at presentation was 14/15. Eye examination revealed bilaterally constricted pupils and spontaneous ocular bobbing ( online supplemental video 1). Ocular bobbing became prominent during attempted upward gaze. There were no focal neurological deficits. Examination of other systems was unremarkable except for bilateral inspiratory crackles.",
