  "abstract": "A man in his 80s with a history of chronic obstructive pulmonary disease (COPD) presented with progressive dyspnoea and purulent sputum. His oxygen saturation was 91% on room air. Physical examination revealed the use of accessory respiratory muscles, such as the prominent hypertrophy of the sternocleidomastoid muscles, and bilateral expiratory wheezes on chest auscultation. Chest radiography demonstrated bilateral flattening of the diaphragms, and CT showed lung emphysema in both lungs. Based on these findings, the patient was diagnosed with an acute exacerbation of COPD.",
