  "abstract": "A male in his late teens was referred to our centre with progressive dyspnoea and declining cardiac function over the past year. He had been diagnosed with Duchenne muscular dystrophy (DMD) at age five, presenting with proximal muscle weakness, positive Gower’s sign and an elevated creatine phosphokinase level of 27 379 mcg/L (reference: 30–200 mcg/dL). Genetic testing confirmed duplication of exon 44 in the dystrophin gene. A transthoracic echocardiogram (TTE) performed 2 years earlier showed normal cardiac size and function. On examination, there was a lateral displacement of the cardiac apex and fine bibasilar crackles. Other findings were unremarkable. N-terminal pro-brain natriuretic peptide was elevated at 5299 pg/mL (normal reference: <300 pg/mL), while high-sensitivity cardiac troponin I was within normal range. A 12-lead ECG demonstrated a normal sinus rhythm with a tall R wave in lead V1 and Q waves in the inferior and lateral leads ( figure 1). Chest radiography revealed cardiomegaly with increased pulmonary vascularity.",
