  "abstract": "A girl in her early teens presented with diminution of vision in the left eye with outward deviation of the same eye since birth. Best corrected visual acuity was 20/20 (−0.5D) right eye (OD), 20/400 (−9 DS) left eye (OS). Orthoptic evaluation revealed an alternate divergent squint of 30 prism dioptres with dominant right eye fixation pattern. Fundus examination revealed bilateral myelinated retinal nerve fibres (MRNF) with extensive myelination in the left eye along with a tilted optic disc ( figure 1). Axial length was 22.89 mm OD and 25.95 mm OS. Taking into consideration unilateral axial myopia, sensory deprivation amblyopia and extensive MRNF, a diagnosis of Straatsma syndrome was made. On systemic evaluation, the patient also gave a history of delayed menarche and abdominal pain for 6 months for which gynaecological referral was advised. Contrast enhanced computed tomography and MRI revealed bilateral ovarian dermoid (figure 2) with right hydrosalpinx for which she was subsequently operated. There was no contributory family or systemic history suggestive of syndromic associations such as neurofibromatosis, tuberous sclerosis or mitochondrial disorders. Neurological history was unremarkable, with no prior episodes of developmental delay, seizures or motor/sensory deficits. Apart from gynaecological details, no other systemic abnormalities were present.",
