  "abstract": "We present a rare case of a middle-aged male with his eyes fixed in hypotropia–esotropia for 12 years. He had been diagnosed with and operated on for posterior fossa pilocytic astrocytoma prior to symptom onset. Contrast-enhanced MRI of the brain and orbit revealed dilated ventricles, a shunt in situ and a distorted midbrain architecture. The recti did not show any displacement, and axial length was not elongated. It was hypothesised that damage to vertical gaze centres, in conjunction with lateral rectus palsy due to protracted hydrocephalus, caused the clinical presentation. Surgical treatment involved bilateral medial rectus recession of 4.5 mm and inferior rectus recession of 7 mm to restore near orthotropia. To the best of our knowledge, this is a novel case of strabismus fixus due to coexisting upgaze palsy and sixth nerve palsy.",
