  "abstract": "A man in his 20s presented after a high-energy MCA. Initially, he was haemodynamically stable; the abdominal exam revealed a large right flank haematoma. On placement of a catheter, there was gross haematuria. CT urography revealed a grade V parenchymal renal injury with a shattered kidney with no active bleeding (blush). After 8 hours of close observation, the patient was vitally stable with a haemoglobin (Hb) of 8.6 gm/dL. Expectant management was chosen with bed rest and close follow-up. Angioembolisation was not recommended because there was no active bleeding (blush) on the CT. On follow-up, subsequent assessments showed normal blood pressure, RF and haemoglobin levels. A follow-up CT urography showed a near-complete recovery of the kidney. This case supports expectant management as a viable option in select cases of grade V parenchymal renal injury, provided that the patient is haemodynamically stable, potentially avoiding the need for nephrectomy.",
