  "abstract": "The hand is famously useful in diagnosis. Think splinter haemorrhages, finger clubbing, or palmar erythema. Hyperpigmentation of the knuckles should be added to the list of things to look for. A 14 year old vegetarian girl with a year’s history of progressive pigmentation over the interphalangeal joints of her hands and feet turned out to be vitamin B12 deficient, despite an absence of gastrointestinal or neurological symptoms.
