  "abstract": "Global geopolitical crises pose multiple threats, including the physical and mental health of those involved, requiring adaptations to optimise well-being in challenging scenarios. The COVID-19 pandemic forced the sports and exercise medicine (SEM) community to construct a rapid-response framework for confinement-induced detraining. 1 More recently, conflicts unfolding across the Middle East and other regions have significantly disrupted the lives of many, affecting their ability to exercise regularly. As ballistic exchanges and strikes near nuclear infrastructure trigger mass shelter-in-place orders, millions of athletes and individuals managing non-communicable diseases (NCDs) are abruptly cut off from physical activity infrastructure. 2 The aim of our editorial is to propose an evidence-based framework that equips the SEM community to mitigate the compounding biological effects of conflict-enforced confinement and, where the worst-case scenario unfolds, concurrent radiation exposure.",
